Specifications include, but are not limited to: This is an outcome based proposal to provide a community intervention in-home behavioral youth program for youth involved with Luzerne County Children and Youth/Juvenile Probation Services. This proposed program serves as an intensive, highly supervised non-residential program designed to treat at-risk youth while they continue to live at home. The services are designed to prevent out-of-home placement, reduce the time in out-of-home placement, and enhance family functioning by improving the safety and well-being of youth and families. The target age population of this program will be from ages 12 to 20 and serve up to 12 juveniles at a time. Program goals should be care coordination, behavior intervention services, pro social skill development, and ensuring community safety. It should utilize evidenced based or evidenced supported curricula whenever possible. The provider will understand criminogenic needs through provided assessments.
