Specifications include, but are not limited to: DHS requests Applications from qualified Applicants to assess Client’s functional, psychosocial, ADL/IADL needs to determine if the Client has medical and ADL needs that require nursing facility supports or if the Client’s needs can be met in the community. The primary goal of the PASRR Level 2 assessment is to determine the most appropriate and least restrictive setting for services delivery for Clients with complex medical, psychosocial, and service needs and to evaluate if they require specialized services. DHS will Contract with multiple Applicants that meet the minimum qualifications, and whose Application receives a score of 70 or higher from the DHS’ Evaluation Committee that evaluates the Applicant’s qualifications, responsibility, and responsiveness. DHS will issue more than one Contract from this solicitation and any subsequent solicitation for these consultation services.
