Specifications include, but are not limited to: Host and participate in monthly conference calls with ODH staff. Communicate with ODH staff by phone on monthly basis regarding the progress and status of each deliverable, barriers, technical assistance (TA) needs and data collection. Submit a monthly progress report, including but not limited to, a monthly TA log, a detailed description of progress and status of each deliverable, as well as any progress made in completing the steps outlines in DPP Expansion Plan. All progress reports should be submitted no later than 2 days prior to each scheduled monthly conference call. In coordination with ODH, conduct a National DPP Landscape Analysis to determine, at minimum, three (3) counties/areas of the state where National DPP implementation would be most impactful over the course of the next three (3) years. The Landscape Analysis should include, but is not limited to, assessing locations of high burden populations, existing CDC-recognized lifestyle change programs, FQHC locations and program demand. The following counties/areas of the state should be considered in the Landscape Analysis as area for expansion, due to highest diabetes prevalence and limited access to the National DP.
