Specifications include, but are not limited to: A. Develop and submit a final workplan for ODH review and approval. The workplan, at a minimum, should include: 1. Specific steps, tasks, and responsible staff members proposed to successfully complete deliverables. 2. Approach and methodology to develop a comprehensive brief specific for Ohio with input from subject matter experts from around the State. 3. Identification of stakeholders. Stakeholders should include but not limited to the following: pediatricians; family practitioners; medical clinics; school administrators; school athletic associations; school-based coaches; school nurses; insurance; Ohio Departments of Education, Medicaid, Mental Health and Addiction Services, Job and Family Services, Insurance; legislators, local health departments, parents/guardians, and adolescents. 4. Stakeholder and recruitment strategies. 5. Locations and dates of forums/meetings. 6. Proposed forum agenda that includes discussion items, questions and facilitation process for forum dialogue to identify needs, gaps and strategies to improve adolescent well visit outcomes. B. Recruit stakeholders invested in adolescent preventive medicine and outcomes to assist the development of improvement strategies that are evidence-informed and feasible for achieving improved measures. Provide ODH with list of stakeholders committed to attend forums. C. Host at least one forum to identify improve strategies and host at least one forum after the development of the brief to release the findings. Additional meetings may be held to assure the necessary stakeholders are able to provide input. Develop agendas for ODH review and comment that promotes engagement and participation in the forums/meetings. Submit list of forum/meeting attendees to ODH. All costs associated with the forums and meetings are the responsibility of the contractor
