Specifications include, but are not limited to: Staff will provide on-site outreach visits to all nursing facilities within their designated service quadrant on a quarterly basis. Outreach activities will include on-site contacts with nursing facility staff, nursing facility consumers, and other area agencies as needed to provide information about MFP transition services and Home and Community Based Services. Center transition coordinators will be expected to explore effective outreach strategies, such as visiting with family councils, resident councils, and one-to-one with facility consumers and staff, as well as professionals in agencies that provide post-transition services, such as Home and Community Based Case Managers
