Specifications include, but are not limited to: Money Follows the Person Nursing Facility Transition Coordination Services North Dakota Money Follows the Person (MFP) Nursing Facility Transition Coordination Services are intended to assist MFP qualified consumers with transition from nursing facilities or hospital settings back into the community. These services are provided through outreach to potential MFP consumers/family members, nursing facility staff, and community agencies, as well as through coordination of transition activities leading to a return to the community. Outreach Activities Staff will provide on-site outreach visits to all nursing facilities within their designated service quadrant on a quarterly basis. Outreach activities will include on-site contacts with nursing facility staff, nursing facility consumers, and other area agencies as needed to provide information about MFP transition services and Home and Community Based Services. Center transition coordinators will be expected to explore effective outreach strategies, such as visiting with family councils, resident councils, and one-to-one with facility consumers and staff, as well as professionals in agencies that provide post-transition services, such as Home and Community Based Case Managers. Vendor will provide Outreach activities in accordance with the terms outlined below. These Outreach services will be provided by either permanent agency staff or full-time MFP Transition Coordination staff. Vendor will be provided funding to create a full-time position(s) that will be dedicated solely to the MFP outreach and transition process activities. The position(s) will provide transition coordination services in accordance with the Duties/Responsibilities listed below and in accordance with the provisions of the most current Nursing Facility Transition Process Guidelines established by the MFP Nursing Facility Transitions Workgroup. These Process Guidelines may be adjusted and modified as needed by the Transition Workgroup. The goal is to transition eight consumes per year from nursing facilities in the quadrant. Reporting A monthly report entitled “MFP Outreach Reporting” will be submitted to the MFP Program Administrator along with the SFN 1763 on a monthly basis. The report will include a list of facilities and agencies contacted, activities included in the visits, and the number of consumer visits within each facility. The MFP Program Administrator will provide the report for use by Vendor.
