The purpose of this proposal is to establish a contract with a healthcare provider (HCP) to administer comprehensive annual physical examinations and fitness evaluations to new and incumbent members of the City of Jacksonville Fire & Emergency Services as we strive to provide for the overall health and wellness of our members.

Detailed proposals outlining how the HCP will satisfy the requirements along with a price per person, per service will need to be submitted to Mary Ann Mitchell, City of Jacksonville Finance Department, by 5 p.m. February 9, 2018 in order to be considered. Proposals may either be emailed to mamitchell@jacksonvillenc.gov or delivered to Mary Ann Mitchell at the Finance Division at Jacksonville City Hall located at 815 New Bridge Street, Jacksonville, NC 28540.
Statewide
