Specifications include, but are not limited to: Information and Assistance provides individualized, person‐centered screening to guide the delivery of long‐term care options and possible services and supports to meet identified needs. Initial telephone contact may consist of a preliminary evaluation of the Older Adults’ and Caregivers’ general social, medical and financial status and the current availability of informal and formal supports. Options counseling is offered as an interactive process, whereby Older Adults and Caregivers are assisted in evaluating their long‐term care options and are supported in making a decision, based on their individual situation. Case Assistance is a subset of Information and Assistance that is more intensive than just providing information and assistance. Case Assistance includes advocating on the behalf of the Older Adults and Caregiver, referrals to programs and services to ensure that delivery of the needed services are provided in a satisfactory manner.
