The Port Chester-Rye Union Free School District (hereafter “the District") is requesting proposals from qualified individuals or firms/agencies (hereafter “The Physician”) to provide physician services to the District’s students for the 2020 – 2021 school year, with an option to renew for the 2021 - 2022, 2022 - 2023, 2023 - 2024 and 2024 – 2025 school years. The District's school year begins July 1st and ends June 30th.
To be considered, one original, marked “Original” plus three (3) copies, marked “Copy” of the proposal must be received by the Purchasing Agent by 11:00 a.m. on June 09, 2020.
