SSN-RC-2019-003 - Multi-Hub Services in Rockland County

The County of Rockland Department of Social Services is considering the issuance of a Sole Source award to WESTCOP- Rockland Community Action Partnership (“Service Provider”) for the provision of Multi-Hub Services to residents of Rockland County.
 
This award may be made on a sole source basis in accordance with the Rockland County Procurement Policy.
 
The Provider shall perform the following Program/Services:
 
Help the client to demystify the social support system and equip low income / high risk clients to negotiate the system in order to progress by offering the following multi-lingual services:
 

1. Direct Services

• housing, Section 8, senior and assisted living
• veterans services, housing, rental assistance and security for new rentals
• social service application / recertification assistance
• school registration applications
• translation services
• services for senior citizens
• Employment and job readiness preparation (resume writing, interview skills and mock interviews)
• Crisis Intervention Services

• 85% of clients enrolled in the Job Readiness Program will complete a resume, cover letter and mock interview.
• 70%* of clients enrolled in the Job Readiness Program will obtain employment within six (6) months of program completion.
• 70% of clients employed will maintain employment for at least one (1) year.

1. Information and Referral Services

• Computer access for job searches
• Child care referrals
• Immigration support
• Referrals to ESL / ESOL classes
• Financial literacy (budgeting, income management, credit repair)
• Job search and application assistance

• Clients receiving counseling and referral services will be referred to the New York State’s MyBenefits website, to ensure that they are informed about various employment supportive services and assistance programs, such as: Temporary Assistance for Needy Families (TANF); Federally Non-Participating Safety Net Employment Program (FNP); Supplemental Nutritional Assistance Employment and Training Program (SNAP E&T).

1. Limited Intervention Services

• Food pantry (breads and other baked goods)
• Clothing
• Essential household items
• Warm weather clothing and blankets
• Back to school supplies

• Total number of individuals or families to be served with the above three categories of services:  7000P
  • Unduplicated** number of individuals or families to be served is 1,400
  • Duplicated number of individuals or families to be served is 5,600

• Services available in both Haverstraw and Spring Valley offices 8 hours per day, Monday – Friday
• Demonstrate successful experience providing the above services to eligible individuals and families by records indicating Performance Target Outcomes have been met

• Collect and maintain records for each client served
• Submit detailed data to the Rockland County Department of Social Services on a quarterly basis for all units of service provided and for all program expenses
• Provider will maintain and submit detailed records indicating all Performance Target Outcomes and contract requirements have been met throughout contract term to DSS upon request
• Provider will maintain and provide copies of its Social Services Resource and Referral Data Collection Instrument, and data and/or documentation that will contain at a minimum:  log sheets for clients served that specifies: date, time, full name of client, phone number, address, type of service(s) received; Provider must provide RCDSS with documentation that specifies the type of information and referral given; specify if a senior citizen and specify how income was verified

•  Provider must include a written attestation that it will be delivering more than one of the following services to all of its clients, as applicable, under the resulting contract:

• Intake, case maintenance and management services* to help families achieve and sustain self-sufficiency
  • *for all federal and non-federal assistance programs; Medicaid, daycare; SNAP
• Child Preventive Services (referral, monitoring supportive and rehabilitative services provided to children under the age of 18 and their families to avert disruption of family unit)
• Child Welfare Services (services to protect and promote the welfare and safety of all children, including individuals with disabilities; homeless, dependent or neglected children)
• Family Preservation Services (services for children and families designed to prevent harm to children and help families at risk or in crisis)
• Family Support Services (community-based services to promote the well-being of children and families designed to increase the strength and stability of families and enhance child development)
• Staff Development Training and Educational programs (services to improve job performance and knowledge on social service programs)
• Refer and Inform Clients about the Home Energy Assistance Program (HEAP)
• For clients receiving employment services, Provider must include a written attestation that all of its clients through a contract with the Department of Social Services will be referred to the New York State's MyBenefits website, to ensure that all clients log on to complete an application and initial screening, informing clients about various employment supportive services and assistance programs, such as: Temporary Assistance for Needy Families (TANF); Federally Non-Participating Safety Net Employment Program (FNP); Supplemental Nutritional Assistance Employment and Training Program (SNAP E&T).

• Must currently be designated as “Community Action Program / Neighborhood Service Center” by the regional WESTCop office
• Must be assigned as an Anti-Poverty Agency by New York State
• Demonstrated successful experience providing the above services to eligible individuals and families by records indicating Performance Target Outcomes have been met
• Must be located in Rockland County
• Must be accessible by public transportation
• Have established services locations in Haverstraw and Spring Valley, NY
• Have multi-lingual staff (English, Spanish, French-Creole, Quechua) to provide services
• Regular attendance at Haverstraw, Spring Valley, Nyack, and Western Ramapo Collaborative meetings
• Must have collaborative agreement with Rockland Housing Action Coalition to provide Financial Literacy Program

• Be a 501 (c)3
• Be governed by a Board of Directors
• Be up-to-date with all IRS filings
• Be up-to-date with all NYS filings
• Have a 2019 budget with Board approval
• Be fiscally sound
• Have a Non-discrimination Policy
• Have a Conflict of Interest Policy
• *Provide a copy of agency’s most recently filed 990 IRS Form

• Maintain program records for the purpose of an audit and provide to the County all program-related information including proprietary materials, upon request
• Be available for announced on-site visits

• Submit quarterly vouchers to RCDSS
• Quarterly Vouchers, which may only include expenses for the respective quarter, that are not supported by the proper documentation and/or not provided to RCDSS within thirty (30) days of the end of each quarter, or within 14 (fourteen days) of request by RCDSS, will not be considered for payment by RCDSS.
