Specifications include, but are not limited to: • Perform Occupational and Physical Therapy Service in accord with the applicable standard of care; • Complete federal and state documentation governing the provision of, and payment for, vendors’ services. • Provide progress reports on the treatment of student(s), Medicaid documentation, or other evaluation reports as may be appropriate or required for each student. Contractor Responsibilities: The Contractor or contractor’s therapists will fulfill the following requirements and perform the following services in a satisfactory and proper manner. The Contractor shall: A. Laptop Computers: Contractor shall provide its own laptop and equipment. APS will not issue laptop computers for work under this proposal. Agencies of contracted Tele practice OTs are responsible for providing equipment. B. Billing: Contractor shall accurately complete invoice logs and invoices for services rendered. Contractor services under this contract are limited to the provision of OT services. Non-OT services must be contracted separately. Contractor shall fully comply with all other state and federal requirements associated with provision of the services contemplated herein and not bill any other party (e.g. Medicaid, 3rd party insurance, or parents). Violation of this provision will result in termination of the contract. C. Mileage: Mileage will not be paid, with the exception of To`hajiilee, Isleta and home bound students and will be calculated from the contractor’s primary office in Albuquerque, NM, with prior approval from the District. If multiple students are served on the same school day, the distance between the current student’s location and the next student’s location will serve as the mileage determination factor. D. Inclement Weather: Scheduled Occupational Therapy service appointments, meetings, or events shall be considered cancelled if APS schools are closed due to inclement weather. Contractor shall not bill APS for inclement weather appointments when the media announces that school will be cancelled for the following day. If school is cancelled the day of inclement weather, hours lost due to delay are not billable. E. Interns: Contractor cannot accept students for internship/practicum experience or accept supervisory duties associated with APS activities.
