Specifications include, but are not limited to: The Contractor shall: • Provide Navigation Services for Santa Fe Magistrate Court defendants (a.k.a. “clients”) ages 18 to 30 years old with serious behavioral health problems. Approximately 20 clients will be participating on a monthly basis. • Participate in Accountable Health Community activities, including attending navigator meetings. • Using the Accountable Health Community Screening Tool (Exhibit A), assess clients’ needs related to social determinants of health, including safe and secure housing; nutritional food; reliable utilities and transportation; and freedom from interpersonal violence. Also assess clients’ needs related to education; clothing; legal documentation; and access to health insurance, medical care, disability services, and mental health and substance use disorder treatment services. • Assess clients’ behavioral health needs including: drug overdose prevention; safety from abuse and/or exploitation; and treatment for disorders related to mental health, substance use, posttraumatic stress, and complex trauma. • Systematically collect, store, and share client and organizational data specified by CSD. Adhere to CSD data collection and use protocols. • Deliver appropriate, quality, and timely navigation services tailored to clients’ diverse cultural and health needs. • Provide navigation services during the time between defendants’ arraignment and their sentencing or final deposition. • Maintain a caseload of no more than thirty clients at once. • Deliver appropriate, quality, and timely health education related to safety, harm reduction, overdose response, and hygiene. • Work collaboratively with extensive network of professional peers invested in clients’ health and safety outcomes including but not limited to: case managers; victim advocates;
