Description

RFP# CC18-62 include an additional team comprised of a Care Navigator and existing Human Service staff. The Care Navigator will be supported through this RFA and staffed by a Morris area nonprofit organization. An additional vehicle will be added to the HOPE One initiative and the two vehicles will go out into the community in tandem or separately depending upon community needs. The intention of the expanded mobile outreach model is to offer the greatest flexibility and autonomy of service provision based on demand. At least one Human Service worker and the Care Navigator will staff the outreach vehicle. The Care Navigator position is Full Time.
