Specifications include, but are not limited to: 3.2.1. The selected Vendor(s) will take primary responsibility for coordinating the day-to-day management of their regional Partners in Health (PIH) Site, in accordance with He-M 523, which consists of assessment, planning, implementation, and on-going evaluation of services delivered. 3.2.2. The selected Vendor(s) must consult with the Department regarding planning, resource location, service design, and coordination of community-based services. 3.2.3. The selected Vendor(s) must contribute to building a strong and healthy community by facilitating families’ social connections, developing their leadership skills, and collaborating with other programs. 3.2.4. The selected Vendor(s) must utilize effective and evidence-based family support practices which include, but are not limited to: 3.2.4.1. Flexible services using the elements of Family Centered Care; 3.2.4.2. Strengths-based assessment and planning; 3.2.4.3. Approaches for behavioral change such as Motivational Interviewing, Coaching, and Person-Centered Planning; and 3.2.4.4. Goal setting using a SMART (Specific, Measurable, Achievable, Realistic, Timely) framework.
