Specifications include, but are not limited to: 3.2.6. The selected vendor(s) must ensure children in out-of-home placement and their families have periodic reviews to ensure progress in improving safety, permanency, and well-being. The selected Vendor(s) must: 3.2.6.1. Conduct a 30-day follow up telephone call, or other method of contact, to the birth families and caregivers in order to obtain updates on the three (3) small manageable tasks identified during the Ice Breaker Meeting. 3.2.6.2. Assist birth parents and caregivers with identifying barriers to successfully complete the three (3) small manageable tasks identified during the Ice Breaker Meeting. 3.2.6.3. Work with birth parents and caregivers to reduce the identified barriers to success.
