Proposal is to contain this entire RFP, cover page through and including page 84, and the Signed Signature page. Submit one original and one (1) copy of the executed proposal in a sealed envelope. Response must also consist of one copy in electronic format on a CD, DVD or USB flash drive formatted in Adobe (.pdf) or Microsoft Word. The electronic copy that is an exact copy of the fully executed original may be submitted at the MITN site for this offering.

Each bidder shall furnish satisfactory evidence of the manufacturer's ability to construct, supply service parts and technical assistance for the apparatus specified. Each bidder must state the location of the factory and location for post delivery service.
