Specifications include, but are not limited to: 1.Undertake activities that identify at-risk, eligible households such as housing and health fairs, door to door canvasing, neighborhood events, mailings, and targeted advertising. HCCW is not prescriptive as to methods and means of identifying target households provided the activity is legal and is not in conflict with the grants policies, the Grant NOFA, 2 CFR 200 and HUD Title X. Activities to identify households should be focused on the grant’s geographical target area and families with pregnant women or newborns are a priority. (See target area map Attachment B). Grant information materials such as flyers and intake forms will be provided by HCCW free of charge. 2. Provide potential grant applicants with grant information and assist with completing the one (1) page grant pre-application/ intake referral form. Collect completed referral forms and send them to HCCW.
