Specifications include, but are not limited to: The Wraparound process shall be based on the individual child and family’s needs and should be literally “wrapped around” the child and family members. Wraparound requires commitments from all involved disciplines. The child and family team members who are determined by the family will collectively do what is necessary to prevent out-of-home placement, facilitate reunification, or de-escalate family crisis. The Wraparound process allows people with diverse beliefs from differing systems and cultures to work together in a way that is meaningful, resulting in positive outcomes for the family. Time – Limited Family Reunification Services shall be provided to Parents and Primary Caregiver(s) who: • are responsible for the care and supervision of minor child(ren) and • have a MDHHS open case in out-of-home placement with family reunification as the goal. • Families are eligible only within the 15 month period that begins on the date the child is considered to have entered foster care. Adoption Promotion and Support Services shall be provided to: • Prospective adoptive parents adopting minor child(ren) through the MDHHS foster care system, when adoption is the child’s permanency goal. • Adoptive parent(s) of minor child(ren) adopted through the MDHHS foster care system, when adoption has been finalized, and a need for post adoption services has been identified by MDHHS. Family Preservation – Placement Prevention Services shall be provided to Parent or Primary Caregiver(s) who: • are responsible for the care and supervision of minor child(ren) and • have an open MDHHS Children’s Protective Services (Category I, II, and III), Foster Care or Delinquency case. Family Support Services shall be provided to Parents or Primary Caregiver(s) who: • are responsible for the care and supervision of minor child(ren) and • have an open MDHHS Children’s Protective Services (Category I, II, and III), Foster Care or Delinquency case or • had an open case that was closed within the past 18 months or • had a CPS investigation within the past 18 month or • had 3 or more rejected CPS complaints.
