Specifications include, but are not limited to: Accountable Care Organizations[footnoteRef:1], health systems, and/or community health centers to develop and implement strategies to increase breast and cervical cancer screening rates and assure appropriate and timely clinical management of abnormal results in specific populations. Populations identified are Black Women, Asian Women, foreign-born Muslim women, and women residing in Health Professional Shortage Areas (HPSA). Organizations will be required to work with a minimum of two of the populations identified and develop community-based partnerships to provide outreach, education, and referral to services in an effort to identify program eligible, low-income women between the ages of 40-64 years.
