Services may be delivered by individual or organizational vendors and are needed statewide. 2101 MEDICAL EVALUATION: A grouping of evaluation services including eye, medical, psychological, audiological, vocational, and low vision examinations and follow-up visits. Services from this program code are delivered to MCB registrants who are active consumers of the Agency's VR program; such services assist consumer and staff develop comprehensive rehabilitation plans. Services may be delivered by individual or organizational vendors and are needed statewide. 2102 DIAGNOSTIC AND EVALUATION: A constellation of diagnostic services intended to assist in the formulation of individualized service plans for Blind consumers. Services from this program code are most often delivered to MCB registrants who are not enrolled in the Agency's VR program. 2103 DIAGNOSTIC AND EVALUATION: A constellation of diagnostic services intended to provide useful information regarding a Blind individual's current and/or projected capacity to engage in work activities. Qualified vendors of this service will provide information regarding work aptitudes, areas of interest and skill levels. Services from this program code are delivered to MCB registrants who are active consumers of the Agency's VR program.
