Specifications include, but are not limited to: CLPPP maintains a multi-tiered strategy to eliminate childhood lead poisoning, focusing on the child and the child’s home environment. A team of three individuals, including a clinical care coordinator, a community health worker, and a code enforcement lead inspector, deliver case management services (CMS) to the family. The goal of CMS is to decrease a child’s blood lead level, eliminate lead hazards in a child’s home, and to prevent future children who may live in the home from being exposed to lead hazards. The community health worker component of CLPPP’s case management model is child centered and includes the child’s caregiver(s). The CHW works to achieve the goal of decreasing a child’s blood lead level and eliminating sources of lead exposure in the child’s environment, while being sensitive to the burden this may create on the caregiver(s). The CHW services include client identification and outreach, individual assessments to identify client needs and responsive resources, and providing linkages to and coordination with appropriate services.
