Specifications include, but are not limited to: Provide Occupational Therapy (OT) Services listed in this solicitation on the premises of the Holly Center, unless other arrangements are pre-approved in writing by the Contract Monitor. Provide supervision of Certified Occupational Therapy Assistant (COTA) staff. Provide regular weekly consultation with the COTA staff related to caseload plans of care. Provide OT Annual Assessment for Current OT Caseload two weeks prior to annual Plan of Care review. Provide all OT Initial Evaluations of new client admissions. Completes OT screening and/or quarterly progress notes as needed. Complete interim OT assessments when the team requests (team includes: Resident Manager/Staff, Physician, Nursing Staff, QA/IS Staff, Social Worker, Guardian; if applicable. Services and work provided under this contract are an estimate of fourteen (14) to fifteen (15) hours per week. Perform additional clinical, consultative, and/or mandated services or activities as needed. Comply with all department directives, state and federal regulations. Utilize available computer applications for all OT documentation reports. Comply with standard clinical documentation requirements of the State and other regulatory agencies.
