Specifications include, but are not limited to: 1.Attend regular status meetings: a.The Contractor will be available for at least weekly status update meetings with MHCC (virtually or in-person) for the duration of the project b.The Principal Investigator and Project Manager will attend the meetings as well as other staff as requested by MHCC 2.Create a detailed work plan for developing the learning and action guide: a.The work plan will include project milestones including key components of this scope of work 3.Assist with care management focus group meetings: a.The Contractor will attend focus group meetings as requested by MHCC, and held about every four to six weeks i.Provide an update in developing the learning and action guide, and obtain input of the work b.The Contractor will use focus group feedback to inform development of the learning and action guide targeted for release in June 2020 4.Develop the learning and action guide: a.The Contractor will conduct a literature review and ongoing research, in consultation with MHCC, to identify existing resources related to care management best practices, effective strategies and regulations b.The Contractor will propose an interactive design concept where empirical evidence is used to reinforce concepts c.The Contractor will draft a detailed outline that includes sections from the design concept and expanded content based on stakeholder feedback, focus group findings, and literature
