Specifications include, but are not limited to: Vendor to provide secure and confidential on-site and off-site destruction of Parish identified documents on an as-needed basis. Vendor to furnish all labor, materials, containers, equipment and supervision necessary to collect and shred/destroy documents from designated Jefferson Parish locations on the East and West Bank of Jefferson Parish. Requesting Department shall submit a copy of the approved Request for Authority to Dispose of Records (SSARC 930 (R 1/12)) with the contract purchase order to the successful bidder. Requesting Department shall submit a blank Certificate of Destruction (SSARC 933 (R 11/2010)) with the contract order and the request for authority to dispose of records will indicate the documents the successful bidder will pick-up to destroy off-site or on-site. The successful contractor must complete the SSARC 933 (R 11/2010)) Certificate of Destruction for each contract order including the approved request for authority to dispose of records form. This certificate and the request for authority to dispose of records must accompany each invoice. Failure to submit will result in delay in payment. Document destruction per pound. The weight of the boxes must be recorded at pick-up and the same weight must be reflected on the invoice. Document destruction per box. The number of boxes must be recorded at pick-up and the same number of boxes must be reflected on the invoice. The shredding process must produce strip-cut shredding or cross-cut pieces. Strips shall be no wider than the NAID AAA Certification guidelines of 5/8” wide. All documents must be destroyed to a non-recoverable form. Shred document pieces maybe recycled. Samples of shredding quality will be furnished to the Parish upon request. Final destroyed documents must be handled in an environmentally friendly manner.
