Specifications include, but are not limited to: RESIDENT EVALUATION AND TREATMENT PLANS JEBWKVC to be responsible for obtaining resident or resident representative consent to treat resident. JEBWKVC to be responsible for obtaining a Physician’s Order prior to the start of treatment. Awarded Vendor will perform mental health examinations and evaluations as needed, based on referrals and consultations with facility physician, administrator, social services, and medical staff. Evaluations must be legible and completed in a timely manner. The mental health provider will provide medication evaluation and written recommendations for prescribed medication to the facility physician. Medication management must document that informed consent has been obtained from the resident/responsible party. Within seventy-two (72) hours after notification by JEBWKVC that a resident has been designated to receive services, awarded vendor will complete and submit a written mental health treatment plan for the resident based on his/her evaluation. All vendor documentation will be made a part of the residents chart in electronic format. In the event you cannot meet the 72 hour timeline, note in your RFP Response, your alternate proposal. (72 hours is the Preferred time-frame) Vendor/Mental Health Provider will review and interview assigned residents and report on findings. This includes review of all data submitted to the facility from external customers to enhance information regarding the resident. Interviews shall include person-to-person contact with the resident to form clinical opinions to develop a course of treatment. Vendor/Mental Health Provider will confer with JEBWKVC staff to evaluate and review individual Residents’ baseline mental health and medical treatment based on aggregate data derived from other staff members’ observations, assessments, description of resident interactions and other notes and determine the course of treatment based upon this data. JEBWKVC agrees to assist in carrying out treatment plan for the resident by providing: staff support necessary to carry out treatment plan, provide location in the facility for awarded vendor to see resident, full access of all medical records and billing information. JEBWKVC agrees to provide access and delivery of medical records if requested for Medicare/Medicaid auditing purposes when required to comply with audits. JEBWKVC will provide a resident list for each visit. The list will identify those residents having behavioral/ psychological issues to be addressed for that visit. The list will identify residents who may be in the hospital, discharged, in hospice, LOA or deceased or out of the facility for any other reason. The list will include any new residents to be evaluated for treatment. JEBWKVC will provide provider a copy of face sheet. Medicare or insurance Card (front and back), Physician’s order for the service and consent for psychological services. JEBWKVC has the right to call vendor between visits when there is a psychological or psychiatric issue. Prior to calling provider, facility will have available resident’s chart, (medications, diagnoses, laboratory reports, etc.) so provider can make decisions and recommendations for an intervention. The Mental Health Provider shall be responsible for providing backup coverage when not available. Mental Health Provider will be available for telephone consultation in the event of a mental health emergency at all times. In the event you cannot meet the requirement of 24/7 availability, note in your RFP response your alternate proposal. 24/7 is the preferred choice.
