Specifications include, but are not limited to: Ensure that all personnel assigned to this project are competent to perform the work. Contractor shall provide credentialing documentation as requested, including, but not limited to: • Have the ability to bill Medicaid and Medicare • Verification of licensure by the appropriate State of Kansas Board. • Nurse Practitioners: Certification by the American Nurse Credentialing Center (ANCC) or other national certifying body. • Professional Liability Insurance: Contractor must maintain a professional liability (medical malpractice) insurance policy with limits of $1,000,000 per claim and $3,000,000 in the annual aggregate covering itself and eligible Providers. • Diplomas, certification of completion of residency/fellowship programs, board certifications, ECFMG, USMLE as appropriate to the position. • Color Copy or Color Scan of Driver’s License and Social Security Card • Current valid DEA certificate • Provide copies of CPR and/or ACLS certification for each contracted employee assigned prior to furnishing of services by each employee; • Determination of at least one (1) year of clinical experienced related to services required; • Applicant interview exploring work history; • Ensure at least three (3) positive work references; • Current health/T.B. and chest X-ray. • Verify employment eligibility as it pertains to the Immigration and Reform Control Act of 1986.
