Specifications include, but are not limited to: BAR CODE SPECIFICATIONS 1. The bar code shall be Code 39 (3 of 9) Symbology. 2. The bar code shall contain 12 digits and shall print in sequential order. 3. Numbering: Forms shall be numbered sequentially, beginning with 300011520001 for Adult Disposition Report Forms and 400010363501 for Juvenile Disposition Report Forms. Contractor is urged to verify numbering information prior to production. The number will appear below the respective bar code, this will be the number displayed when read by a bar code reader. 4. The bar code shall be placed in upper right hand portion as shown on the attached samples. Human-readable digits shall be no less than 1/9“ in height. Bar Code height and width (with ¼” quiet zone on all four (4) sides) to be as large as the provided space will allow. 5. Bar code shall be printed using ink that is black only and of the best quality. Scanning resolution-300 dpi. No impact printing accepted. 6. The bar code and human-readable digits will appear on parts One, Two and Three of each form. 7. The bottom of each page notates instructions in on who receives the form. First Copy – Central Repository. Second Copy – Central Repository. Third Copy – Prosecutor.
