Specifications include, but are not limited to: 1. Conduct an independent physician peer review of all deaths using the following format: a. Review of past medical history, diagnoses, and events leading up to the death. b. List of treatments prior to transfer to the Emergency Room. c. Diagnostic assessments, evaluations, and test performed during the terminal illness. d. Cause of death e. Pre-mortem diagnosis consistent with the cause of death. f. Was medical treatment appropriate and consistent with current standards. g. Was an autopsy examination performed. h. Suggestions on how to improve medical services. 2. Peer review must be done by a licensed physician who is not employed by Glenwood Resource Center. 3. The reviewer will have available the complete institutional record of the individual, the report of the investigator, the physician's review, the nursing review, any report of the mortality review committee, and any other documents or information the reviewer believes to be relevant. 4. The reviewer will provide a report based on the evaluation of records and identify any recommendations for opportunities for improvement in the quality of care being provided.
