Specifications include, but are not limited to: • The main program activities of linkage to care are to: a) hire appropriate individuals to conduct outreach, engagement, and linkage to care; (b) conduct outreach, engagement, and linkage to care for individuals at greatest risk of experiencing overdose death due to substance use (c) engage in quality improvement, reporting, and training; and (d) collaborate with CDPH on citywide initiatives related to substance use prevention and treatment. • Work with CDPH to establish funding and resource allocation priorities that will maximize the funds available to support the delivery of linkage to care programs to vulnerable populations in Chicago. • Assure that funding is used primarily to support programs and services for the uninsured or underinsured. Insured residents seeking services available through this funding may do so if their insurance plan does not cover the requested service. • Engage in ongoing quality management activities, including developing and reviewing monthly reports for CDPH and engaging in annual fiscal and programmatic site visits by CDPH.
