Specifications include, but are not limited to: 1. Review results of standardized screening tools administered by HPD personnel that are utilized to identify persons with medical and behavioral health needs (including psychiatric and substance use disorders). 2. For those who screen positive for a behavioral health condition, provide assessments, evaluations, and care to include: a. Reviewing medication needs, prescribing medication, and assisting with medication administration, including Medication Assisted Treatment (“MAT”) for substance withdrawal; and b. Initiating timely referral to appropriate community mental health and SUD services via Hawaii CARES upon discharge. The Hawaii CARES program is the DEPARTMENT’s Behavioral Health Administration’s one-stop access line for SUD treatment. 3. For those who screen positive for an acute physical health condition, provide assessment and care to include: a. Assessing medical needs and assisting triaging for referral to an emergency department for further medical evaluation; b. Reviewing medication needs, prescribing medication, assisting with medication administration; and c. Administering first aid. 4. Communicate with DIVISION’s court-based clinician, jail diversion staff, and assigned case management service providers when known and appropriate, to support jail diversion activities including services via Hawaii CARES.
