Specifications include, but are not limited to: The PPO dental plan shall have the following features: a. The design shall have a $50.00 deductible for all coverage except preventive. b. The preventive care benefit shall be paid at 100% with the $50.00 deductible waived. c. The basic care benefit shall be paid at 80% with the $50.00 deductible applying. d. The major care benefit shall be paid at 50% with the $50.00 deductible applying. e. The orthodontia benefit shall be paid at 50% with $50.00 deductible applying f. The calendar year maximum for preventive, basic and major shall be $1,500.00 per year. g. The lifetime maximum for orthodontia shall be $1000.00. h. Dependents covered up to age 26 i. The preventive, basic, major and orthodontia shall be paid in accordance with the insurance industries reasonable and customary standards. j. Claim disputes shall be resolved within sixty (60) days. k. There shall be information readily available to the Director of Benefits and Risk management for new hires and open enrollment on a monthly and annual basis respectively for dental benefit. l. The contract shall include extension of benefits for enrollees if the contract is terminated. Please include your contract language for this provision. m. Membership deductions shall be provided monthly on file transfer basis. n. The vendor shall provide APS with the necessary informational material for conducting enrollment meetings, along with related servicing personnel every open enrollment period prior to plan anniversary. All employees of the vendor engaged in enrolling employees in the plan and providing continuing service with respect to the plan shall possess all licenses and permits as may be required by state or federal law. o. The vendor shall agree to perform all solicitations in a restrained manner without any pressure, harassment or disruption of the normal business activities of APS. p. The vendor’s proposal shall provide a COBRA benefit for retirees up to an 18 month period. q. The vendor’s proposal shall provide for the missing tooth exclusion. r. The vendor shall assume responsibility for all claims incurred on and after the effective date.
