Specifications include, but are not limited to: Contractor shall provide an example comprehensive medical questionnaire upon County's request. HCFR shall approve the comprehensive medical questionnaire. This will be developed between the Health and Wellness Chief and the Contractor. HCFR shall distribute through email the pre-approved comprehensive medical questionnaire to all participating personnel seventy-two (72) hours in advance of the physical examination. Contractor shall schedule and perform all blood draws in advance of the examination and assessment. The results of both shall be available to the healthcare practitioner at the time of the exam and assessment. HCFR shall schedule all examinations, assessments, and related consultations. Contractor shall perform the following tasks: Comprehensive Physical Examinations (Per National Fire Protection Agency (NFPA) 1500 and 1582); Health Risk Appraisal; Prostate Exam for Men; Pelvic Exam for Women; Breast Exam for Women; Skin Cancer Screening; Mental Health Assessment, to be conducted by a clinical Psychiatrist and/or Nurse Practitioner of Psychiatry; Primary Care Post-Traumatic Stress Disorder (PTSD) Screen for Diagnostic and Statistical Manual of Mental Disorders (DSM-5).
