Specifications include, but are not limited to: 1. Collaborate with the DDOC’s contracted behavioral health/medical provider and DDOC staff to ensure continuity of care for offenders having high risk/high need challenges. The Provider will collaborate with the Healthcare and Behavioral Health provider under contract with DDOC and DDOC staff providing execution of discharge plans for individuals being released from Level V and Level IV facilities that require continuity of care and additional supports. The Provider will assist individuals in implementing their individual plans upon release. 2. Assign/identify a minimum of one (1) staff person to participate in monthly discharge planning meetings in Level V and Level IV facilities. 3. Provide needed transportation to reentrants, as identified by the contracted medical/behavioral health provider, and DDOC staff responsible for discharge planning. The transportation destinations may include, but are not limited to: Providing transport upon release to predetermined residence or inpatient treatment. Transport to Probation & Parole, as needed.
