Specifications include, but are not limited to: The three (3) main goals are to: 1. Provide dental sealant services to children with a focus on first and second graders with erupted first molars and sixth and seventh graders with erupted second molars; 2. Expand the SSP to at least three (3) eligible schools where applicants are not currently providing sealant services. Eligible schools are defined as schools where 50% or more of students are eligible for the FARM program; and 3. Collect and report SSP data, per CDC’s Sealant Efficiency Assessment for Locals and States (SEALS) data requirements, to the Department on a quarterly basis and an annual submission to CDC SEALS. The contractors identified through this RFP will work with the Department to accomplish program goals as outlined in the CDC grant requirements. Each of the applicants will be required to address the following activities: • Sealant placement; • Quality assurance and retention; • Infection prevention and control; • Referral for follow up and urgent care needs; • Oral health education; and • Data collection and reporting.
