Description

Project Description: Thornton intends to select Occupational Physician Providers that will be made available to employees injured on the job, and will provide prompt, quality industrial/occupational medical care which will enable employees to recover from their injuries and return to gainful employment as soon as possible. The initial term of the Agreement will begin on January 1, 2019, and shall terminate on December 31, 2019; however; Thornton shall have the sole option of renewing the Agreement for up to four (4) additional one (1) year terms.

The City of Thornton, CO, (“Thornton”) respectfully requests separate sealed Proposals to provide Occupational Physician Services, Project No. 189-18 (hereinafter referred to as “Project”). Proposals will be received until 4:00 PM, local time, September 13, 2018, in the Purchasing Office, 9500 Civic Center Drive, 2nd Floor City Hall, Thornton, CO 80229-4326.
