This Request for Proposals (RFP) is for the: 1) receipt of lead test kit requests from Denver Water, assembly oflead test kits, and delivery of lead test kits to Denver Water customers; 2) program lead test kit return deliveryand/or pick-up from Denver Water customers, laboratory analysis of lead test kits, laboratory sample datacompilation, quality assurance/quality control (QA/QC) of laboratory sampling results, and laboratory sampleresults reporting to Denver Water; 3) program laboratory sample results hard copy letter generation and delivery toDenver Water customers with an electronic copy to Denver Water for Denver Water’s Lead Reduction Program(LRP); 4) pre-lead service line replacement (LSLR) laboratory analysis of lead test kits, laboratory sample datacompilation, quality assurance/quality control (QA/QC) of laboratory sampling results, and laboratory sampleresults reporting to Denver Water; and 5) Pre-LSLR laboratory sample results hard copy letter generation anddelivery to Denver Water customers with an electronic copy to Denver Water for Denver Water’s Lead ReductionProgram (LRP).Denver Water is seeking a turnkey proposal inclusive of the five functions; however, the five functions may not beavailable from one source. For this reason, the functions are divided into five schedules. Proposers may submit aproposal for one, some, or all of the schedules.
