Specifications include, but are not limited to: The Santa Clara County Behavioral Health Services Department (BHSD) is seeking proposals from residential care providers to provide board and care and supplemental services for adult/older adult residents of Santa Clara County. Clients are to be transitioned from a higher level of care to the safest, least restrictive, and most appropriate residential setting based on their ability to function independently. The requested services are below: a. Basic Supplemental Residential Care Services – Clients under this category are those who are dual diagnosed as seriously mentally ill (SMI) in combination with some medical conditions that preclude them from being placed in a regular, independent Residential Care Facility (RCF). (Attachment A1) b. Intermediate Residential Care Services – Clients under this category are those with significant difficult mental health and medical conditions that interfere with standard RCF placement. These clients are diagnosed as SMI with significant co-morbid medical conditions and have a level of functioning, symptoms, and psychiatric history that necessitates supplemental service interventions. (Attachment A2) c. Supplemental Services for SMI clients with Traumatic Brain Injury (TBI) – Clients under this category are those diagnosed as SMI with co-morbid conditions of TBI, anoxia (absence of oxygen in the brain), stroke or seizure disorders. Clients may be vulnerable/at risk of victimization in a community setting and may or may not be receiving Supplemental Security Income benefits. (Attachment A3).
