Specifications include, but are not limited to: A. Contractor’s services shall include, but are not limited to: evaluation, management, examinations, consultations, general, regional, spinal, epidural, and local anesthesia, and any interventional or diagnostic procedures pertinent to anesthesiology services. B. Contractor shall provide services on an as-needed basis, not to exceed nine (9) sessions a week. A “session” is defined as one (1) patient undergoing an ECT procedure in one (1) day. Each patient shall not exceed three (3) sessions per week. C. Contractor shall include a copy of the anesthesiologist reports in patient’s medical records. D. Contractor shall provide instruction sheet, examination record, diagnostic test, imaging results, diagnosis, treatment, and follow-up recommendations for any DSH patient served. E. Physicians providing services under this Agreement shall be licensed by the California Department of Consumer Affairs, Medical Board of California with board certification in anesthesia. F. Prior to the provision of services to patients of DSH, Contractor’s physicians shall apply for and be granted privileges as members of the organized medical staff of DSH-Metropolitan, or shall authorize DSH to verify said Contractor’s credentialing and privileging status at a distant site, as requested by DSH.
