Specifications include, but are not limited to: 1. Integrated Primary Health Care for Older Adults living in the Tenderloin SFHN Primary Care seeks proposals to provide comprehensive, integrated primary health care to older adults, aged 55 and older, who live in the Tenderloin and South of Market areas, to promote their health and to maintain living independently in the community. The target population for this primary health care service includes un-insured and under-insured low income older adults. Primary care clients in the Tenderloin and South of Market are ethnically and racially diverse, many are multiply diagnosed (with substance abuse and/or mental health problems in addition to acute and chronic medical conditions); some of whom are non-English speaking and homeless. The services to be provided include billable primary care visits, prevention, early detection, treatment, service coordination and referrals. This program will receive patient referrals, as well as conduct outreach to these provider networks and service partners to recruit clients. 2. Primary Care Health Services for Residents of the Mission Neighborhood SFHN Primary Care invites qualified proposers to submit a proposal to provide comprehensive primary care health services to eligible patients in the Mission District of San Francisco under the Medical Assistance Program (MAP). The overall goal of MAP services is to improve the health status and wellbeing of patients by providing comprehensive primary medical care. This is achieved by providing onestop access to routine health care, episodic or chronic medical screening and examination, medical diagnostic testing, and treatment. Individuals eligible for MAP services include persons who reside in San Francisco and who are not eligible for Medi-Cal or Medicare or who are not subscribers in a San Francisco Managed Care Program or insured by any private insurance program. 3. Community Health Programs for Youth – Sexual Health Education, Substance Abuse Prevention, and Operational Support for a SFHN Primary and Reproductive Care Clinic for Youth The SFHN Community Health Programs for Youth (CHPY) seeks proposals to provide comprehensive sexual health education, HIV prevention, healthy relationship exploration and substance abuse prevention to youth ages 12-18 in San Francisco. Health education and outreach efforts will target youth ages 12-18 in San Francisco Unified School District (SFUSD) middle and high schools, charter schools, continuation schools, community-based organizations, the Juvenile Justice Center, street fairs, health fairs and other Huckleberry Youth Programs. In addition, this program will provide operational support to at least one SFDPH primary and reproductive care youth clinic.
