Specifications include, but are not limited to: Providing the Riverside County Office on Aging (RCOoA) with a Human Services Case Management System for the Aging Network to replace the current system utilized. One of the major takes of providing the software service is the provision of a case management system that requires little to no customization. RCOoA provides social service programs that assist Riverside County’s older adult population to live safely while maintaining as much independence as possible. Riverside County Social Service Practitioners work with their clients to identify service needs and to develop a service plan to meet those needs. The service plan often requires coordination of services from a wide variety of agencies including Department of Public Social Services (DPSS) Adult Services Division, Public Authority In Home Supportive Services, Riverside County Behavioral Health, Riverside University Health Systems and volunteer and community based organizations. RCOoA also coordinates countywide multidisciplinary teams which allows for the implementation of complex, multi-agency service plans. The System shall be a comprehensive, multifunctional system used for data collection for care management clients and service units. It shall support services for Area Agency on Aging (AAA), Older Americans Act (OAA) and Older Californians Act (OCA) programs and services, which includes in-house programs such as the Multi-purpose Senior Services Program (MSSP). The Software Application selected must provide: a client tracking system that addresses National Aging Program Information Systems (NAPIS) and California Aging Reporting System (CARS) requirements; for fundamental case management tools; a scalable, flexible and secure Information and Referral (I&R) data management system that incorporates both a central web-accessible I&R data repository through URL development and individual I&R services, with minimal customization. The I&R database must support and include the Resource, Content and Consumer groups of data. All software and services must be provided through one single vendor. It is the intent of the County to award an agreement to one qualified company as a result of this solicitation which contain the minimum requirements and all terms and conditions for the software service.
