Specifications include, but are not limited to: 3.1 Contractor shall maintain HSAs for individuals who can be associated with the County for aggregate reporting and reconciliation purposes; 3.2 Contractor shall maintain account holder records reflected as the assets of the HSAs and all activity within the accounts for a minimum of 2 years after the accounts are closed; 3.3 Contractor shall prepare tax reporting and provide applicable tax forms (1099SA, 5498 SA) for all individual account holders; 3.4 Contractor shall provide enrollment support specific to County of San Diego’s plan design, including welcome packets, handbooks, and FAQ documents; 3.5 Contractor shall agree to pre-enrollment counseling for potential participants; 3.6 Contractor shall agree to support the County’s wellness program including periodic activities (i.e. lunch & learns) or campaigns in which it actively partners with the County to promote the County’s wellness program; 3.7 Contractor shall agree to support the County’s open enrollment activities by attending open enrollment fairs and presentations; 3.8 Contractor shall notify participants and the County of any pending items needed to establish their HSA; 3.9 Contractor shall accept and process contributions from the County and Account Holders, including transfers from other institutions, up to the maximum amount allowed by law. County and employee contributions shall be processed and posted within 24 hours of receipt; 3.10 Contractor shall provide and maintain an online portal with adequate security for both participant and employer account management, including employer reporting capabilities, such as account status reports, reconciliation reports, etc.; 3.11 Contractor shall provide services in accordance with eligibility and benefit information provided by County staff; 3.12 Contractor shall provide a process to re-affiliate an account for an employee who has become unaffiliated in error. The process shall not require the permission from the HSA owner (the employee); 3.13 Contractor shall continue an affiliated account associated with the County of San Diego and shall not create a separate and additional account; 3.14 Contractor shall agree that any account opened through the County of San Diego and not officially terminated will be considered open regardless of the balance; 3.15 Contractor shall not require a minimum balance in the account to hold it open after an individual is no longer enrolled in a high deductible health plan (HDHP); 3.16 Contractor shall identify contributions and administrative fee payments separately when reporting information to the account holder.
