Specifications include, but are not limited to: The County of San Diego is seeking proposals from firms interested in providing case management services to individuals sixty (60) years and older who are experiencing homelessness with the goal of achieving stable housing and successful independent living. Clients served in this program shall undergo a physical and mental health assessment and receive referrals for addressing medical needs. In addition, clients shall be assisted in accessing benefits they could be entitled to, such as, pension, Social Security/ Supplemental Security Income, and/or Medicare/Medi-cal. Participants shall also be provided with temporary housing for the duration of their program participation, helping ensure their success to achieve stabilize housing.
