Specifications include, but are not limited to: Contractor shall provide Achievement Center location(s) and associated services for youth referred by Probation, including youth who are at-risk of not completing all court-ordered requirements. Target Population 3.1. Contractor shall serve youth ages up to age nineteen (19) as referred by Probation. This may include youth out of compliance with Probation. 3.2. Contractor shall first serve youth on probation then if space is available will next serve the youth’s family and friends and if additional space is available may also serve others in the community who are referred by another community organization. 3.3. Contractor shall serve up to 25 youth at a time and shall not decline a probation youth referral or bump an actively participating probation youth for non-probation youth. Contractor shall use their best discretion when determining how many non-probation clients can participate. Goals and Outcomes 4.1. Referrals: 95% of youth referrals will be contacted within two (2) business days. 4.2. Successful Completion: 4.2.1. 90% of youth participating in the program will not have a new true finding. 4.2.2. 75% of youth will not be detained for a probation violation or have a true finding six (6) months after program completion. 4.3. Participant Satisfaction: 4.3.1. Contractor shall collect formal feedback on service delivery and use the data to inform programming. This will include, but is not limited to: quarterly surveys, exit surveys/interviews, and/or communication with youth and their families. Survey shall be pre-approved by the COR. 4.3.2. 80% of youth successfully exiting the program will complete a satisfaction survey within thirty (30) days after youth exits program. 4.3.3. 80% of the parents/guardians of youth successfully exiting the program will complete a satisfaction survey within thirty (30) days after youth exits program.
