Specifications include, but are not limited to: a. The intent of the Title III-D Evidenced-Based Program for Disease Prevention (EBPDP) is to provide older adults 60+ with services that will: Improve health and promote healthy life styles; Reduce the impact that physical disorders have on the quality of life of older adults; Promote and engage older adults in exercise and other health related activities; and Address issues of incorrect medication and adverse drug reactions. The Contractor is required to tailor an evidence-based service plan to the needs of the participants’ representative of each of the respective Aging Service Areas. b. The successful proposer will provide EBPDP programs from the LADOA approved list (below). The list may be updated as new programs become available. The successful proposer must provide a minimum of one evidence-based program from each of the two categories listed below: Self-Management Programs: Chronic Disease Self-Management/Healthier Living Healthier Living – English version Tomando Control de su Salud – Spanish version Physical Activity programs: A Matter of Balance (English and Spanish) Active Start (English and Spanish) Aquatics Exercise ProgramSM (English and Spanish) Arthritis Foundation Walk With Ease ProgramSM b. In addition, the contractor may provide programming activities that reflect a combination of health screening, medication management, education, counseling and other activities that promote physical fitness, falls prevention, and emotional well-being. Services are directed to prevent and mitigate the consequences of chronic diseases in participants with an emphasis on evidence-based health promotion programs.
