TAY Full-Service Partnership (FSP) Individuals with Serious Mental Illness (SMI) or Severe Emotional Disturbance (SED), chronic mental health and co-occurring conditions Intensive Services 2. TAY Early Psychosis Intervention and Recovery TAY at risk of or experiencing onset of psychosis Outpatient 3. TAY Community-Focused Engagement and Treatment to all populations, including: a. Black/African American b. Asian and Pacific Islanders c. Latino, Mayan and Indigenous d. LGBTQ+ Black/African American, Asian and Pacific Islanders, Latino, Mayan and Indigenous, LGBTQ+ Low Threshold 4. Behavioral Health Services for TAY Experiencing Homelessness a. TAY Homeless Mobile Treatment Team. b. The Coordinated Entry Youth Access Point (CE YAP) Behavioral Health Clinician At risk of or experiencing homelessness and residing at various TAY identified sites Low Threshold 5. TAY Acute Linkage Clients discharged from crisis services who need support with discharge plans or linkage to appropriate levels of care Linkage & Systems Care Coordination 6. TAY Linkage: Behavioral Health Linkage Team for TAY Experiencing Homelessness and in Permanent Supportive Housing (PSH) and focused referral sites TAY at risk of or experiencing homelessness and in permanent supportive housing or referral sites Linkage & Systems Care Coordination
