Specifications include, but are not limited to: The focus of this program is to provide intensive services and supports, as needed, and strong connections with homelessspecific community organizations. Outpatient Services 1. To locate and engage homeless and runaway SED children/youth and SMI Transitional Age Youth (TAY) for the purpose of increasing access to mental health services including co-occurring disorders treatment. 2. To offer integrated services with an emphasis on whole-person wellness and promote access to medical, social, rehabilitative and other community services and supports. 3. To provide supportive services designed to assist with continuity of treatment, linkage to resources, and transition between programs and to community resources. Shelter Bed Component 1. To provide short-term (up to 30 days) housing that incorporates innovative housing in two (2) HHSA regions of the County and supportive services specifically tailored to meet the needs of homeless youth ages 12 through 17. This component may operate as part of the certified outpatient behavioral health program or stand alone. 2. To provide shelter and support services (i.e. case management) to homeless youth with the purpose of providing immediate shelter and to support transition to housing stabilization.
