Specifications include, but are not limited to: • Make outreach and engagement efforts to increase trust, awareness, responsiveness, and acceptance of behavioral health services in the elderly community. • Help ensure both older adults and their families know the resources available to them. • Prepare and deploy engagement strategies that may include offering in-home and infacility visits, socialization, counseling, resources and referrals. • Build resiliency in the older adult population by fostering community involvement, social connectedness, and other protective factors. • Provide support for older adults and their families who are facing specific challenges such as grief, suicide risk, isolation, and trauma using evidence-based practices • Foster partnerships between clinical staff, peer staff, and/or volunteers, utilizing each partner’s expertise to provide outreach, engage, assess, and connect individuals and families with appropriate services. Also, provide referral and linkage to treatment and/or appropriate community resources • Utilize technology, attendance at older adult events, and other outreach strategies to bring support and information to those that need it, rather than having them seek it out.
