Specifications include, but are not limited to: 3.0.1 Contractor(s) shall schedule the referred MOA employee for an appointment within three (3) working days from the MOA Administrator’s request, unless the MOA Administrator agrees to a later date. 3.0.2 Contractor(s) shall meet with the MOA employee (in person) at the Contractor(s) Anchorage office. 3.0.3 Contractor(s) shall evaluate the employee including a psychiatric assessment, mental status examination, psychological testing, and other evaluations and/or tests as necessary. 3.0.4 Contractor(s) shall review any pertinent medical, employment, or other background files as necessary, furnished by the MOA Administrator, to reach a diagnosis. 3.0.5 Contractor(s) shall verbally report findings to the MOA Administrator within three (3) working days of concluding the employee evaluation, to include diagnosis and recommendations addressing fitness for duty or second opinion evaluations. 3.0.6 Contractor(s) shall document findings of each evaluated employee with a full and final detailed written report delivered to MOA within five (5) working days of the verbal discussion with MOA Administrator. 3.0.7 Contractor(s) shall send MOA a bill for services for each employee on or after the final report delivery date. 3.0.8 Contractor(s) must decline services and recommend to MOA another psychiatric or psychological counselor if any referred employee is: 1) a post client of Contractor; 2) known personally to the contractor; 3) related to the contractor and/or; 4) in any way detrimental to the employer or the evaluations because of any perceived conflict of interest with the contractor. 3.0.9 The intent of this contract is not to provide or encourage any long term or ongoing psychological/psychiatric care or rehabilitation for the employee. This contract should not create a doctor patient relationship.
