Specifications include, but are not limited to: A. Provide back-up dietician services for the facility within 48 hours of the notice. B. Provide nutritional assessments, re-assessments, reviews, and monthly notes in the medical records of nutritionally high-risk geriatric patients in a timely manner. C. Participate in patient treatment planning conferences as needed for consultation. D. Provide weekly monitoring of all patients units (Unit CA, Unit CB, Unit DA, and Unit DB) and in dining room during breakfast, lunch, and supper meals. E. Provide weekly monitoring of patient diet lists to determine accuracy of diet and nutritional supplements and compliance of snacks. F. Communicate with Certified Registered Nurse Practitioner or Medical Director as needed for high-risk patients with changes and/or recommendations. G. Assess tube fed patients and document assessments in their medical records on a weekly basis.
