Specifications include, but are not limited to: Intake Triage Personnel Performance Requirements: The contractor shall have an intake triage personnel answer the initial call and take basic information and facts from the injured team member. The contractor’s intake triage personnel’s process shall include obtaining the injured team member’s personal and generic details from either the injured team member, their supervisors, or other individuals familiar with the alleged injury. At a minimum, the following information listed below must be obtained and documented during each initial call. This information can also be found on Attachment 1, Telephone Injury Report Form. Date of Call; Start and end time of call, documented in central standard time; Injured team member name; Caller name, if not injured team member; Injured team member date of birth; Injured team member age; Injured team member gender; Injured team member phone number; Injured team member email; Department and agency/facility of injured team member; Date and time of incident; Location of where the incident occurred; Injured team member’s supervisor’s name and statement (if available for such); Witness names; Additional information as requested by the state agency. The contractor’s intake triage personnel shall answer one hundred percent (100%) of calls received. Calls shall not be forwarded to voicemail. The contractor’s intake triage personnel shall not put the caller on hold for longer than three (3) minutes.
